Introdução: O traumatismo dentário é uma ocorrência frequente durante a infância e adolescência. Geralmente, o diagnóstico acontece de forma tardia, devido à falta de conhecimento da população. Cada tipo de trauma necessita de uma conduta única e especializada. Nos casos de comprometimento pulpar, é necessário cuidado quanto à escolha da técnica a ser empregada, bem como a seleção da medicação intracanal que, por sua vez, apresenta grande interferência no sucesso do tratamento. Não obstante o avanço das técnicas de instrumentação, a troca da medicação entre as sessões nesses casos é mandatória. Relato de caso: Um paciente, sexo masculino, em idade escolar (15 anos) apresentou-se ao serviço de saúde bucal com relato de escurecimento e dores na mastigação após trauma dentário durante a infância. Os testes de vitalidade pulpar revelaram necrose pulpar, com resultado negativo no teste térmico e positivo na percussão. O exame radiológico evidenciou uma lesão apical, juntamente com reabsorção externa no elemento 21, confirmado em tomografia. O paciente foi submetido a um tratamento endodôntico. Resultado: Após o tratamento, foi solicitado novo exame tomográfico, em que se evidenciou um processo de cicatrização óssea no periápice e reabsorção por substituição na lesão cervical. Conclusão: A partir deste caso clínico, pôde-se evidenciar a importância do diagnóstico adequado, do manejo e da técnica endodôntica eficaz na resolução de problemas decorrentes de traumas dentários em pacientes jovens.


Introduction: Dental trauma is a frequent occurrence during childhood and adolescence. Generally, diagnosis happens late, due to lack of knowledge among the population. Each type of trauma requires a unique and specialized approach. In cases of pulp involvement, care must be taken when choosing the technique to be used, as well as the selection of intracanal medication, which, in turn, has a major impact on the success of the treatment. Despite advances in instrumentation techniques, changing medication between sessions in these cases is mandatory. Case report: A male patient of school age (15 years old) presented to the oral health service with reports of darkening and pain when chewing following dental trauma during childhood. Pulp vitality tests revealed pulp necrosis, with a negative result in the thermal test and positive in percussion. The radiological examination showed an apical lesion, together with external resorption in element 21, confirmed on tomography. The patient underwent endodontic treatment. Result: After treatment, a new tomographic examination was requested, which showed a process of bone healing in the periapex and resorption by replacement in the cervical lesion. Conclusion: From this clinical case, it was possible to highlight the importance of adequate diagnosis, management, and effective endodontic technique in solving problems arising from dental trauma in young patients.

ABSTRACT

Objective: to analyze, through literature review, the available literature on orthodontic treatment in traumatized permanent teeth. Methods: A literature search was performed in electronic databases (PubMed and SciELO) using the descriptors [tooth injuries] OR [injuries, teeth] OR [injury, teeth] OR [teeth injury] OR [injuries, tooth] OR [injury, tooth] OR [tooth injury] OR [teeth injuries] AND [orthodontics]. Observational studies and clinical trials were included, narrative reviews, laboratory and in vitro studies, case reports and series as well as articles that presented abstracts written in languages other than Portuguese, English and Spanish were excluded from the study. Two reviewers considered the eligibility, the risk of bias of the analyzed data and the qualitative synthesis of the studies included. A total of 1,322 references were found and 4 articles met all inclusion criteria and were included in the qualitative analysis. Some consequences like pulp necrosis and root resorption have been highlighted and trauma severity should be considered when orthodontically intervening in previously traumatized teeth. Final considerations: The traumatized teeth can be orthodontically treated as long as the time of tissue reorganization is respected, and the pull and periodontal conditions are followed up.(AU)


Objetivos: analisar, por meio de revisão de literatura, a respeito do tratamento ortodôntico em dentes permanentes traumatizados. Metodologia: Uma pesquisa bibliográfica foi realizada em bancos de dados eletrônicos (PubMed e SciELO) usando os descritores [tooth injuries] ou [injuries, teeth] ou [injury, teeth] ou [teeth injury] ou [injuries, tooth] ou [injury, tooth] ou [tooth injury] ou [teeth injuries] e [orthodontics]. Foram incluídos estudos observacionais e ensaios clínicos, revisões narrativas, estudos laboratoriais e in vitro, relatos de casos e séries, bem como artigos que apresentassem resumos redigidos em idiomas diferentes do português, inglês e espanhol foram excluídos do estudo. Dois revisores consideraram a elegibilidade, o risco de viés dos dados analisados e a síntese qualitativa dos estudos incluídos. Foram encontradas 1.322 referências e 4 artigos atenderam a todos os critérios de inclusão e foram incluídos na análise qualitativa. Algumas consequências como necrose pulpar e reabsorção radicular têm sido destacadas e a gravidade do trauma deve ser considerada na intervenção ortodôntica em dentes previamente traumatizados. Considerações finais: Os estudos incluídos nesta revisão sugerem que dentes traumatizados podem ser tratados ortodônticamente desde que respeitado o tempo de reorganização tecidual e acompanhadas as condições pulpares e periodontais.(AU)

El trauma dentoalveolar se define como aquella lesión de extensión e intensidad variable y de ori- gen accidental o intencional, causada por fuerzas que actúan sobre el órgano dentario y los tejidos que lo rodean. Se puede diagnosticar a simple vista o con ayuda de la radiografía, se debe tratar sin demora para preservar la vitalidad de la pulpa, sobre todo en casos de niños y adolescentes, to- mando en cuenta la formación radicular. El trau- ma dental es motivo de urgencia por el dolor que provocan en los pacientes, la prevalencia varia del 26 al 76% de las lesiones dentales en dentición permanente (AU)

ABSTRACT

Stabilization with splint is an important surgical technology in traumatic dental injuries of permanent tooth. In order to standardize the clinical application of the technique and to improve the therapeutic effects of traumatic injured teeth, the Society of Stomatological Emergency, Chinese Stomatological Association organized relevant professional experts and put forward an expert consensus on the basis of considerable discussion. The contents of the present expert consensus covered indications for tooth fixation, material selection of splints and operation methods, applying to the treatment of tooth loosening, tooth displacement and tooth avulsion replantation caused by traumatic injuries. It is also suitable for preoperative and prognosis evaluations of traumatic injured teeth before tooth fixation.

ABSTRACT

This retrospective observational study aimed to analyze cases assisted at the Dentoalveolar Trauma Clinic of the School of Dentistry of the Federal University of Rio Grande do Sul (FO-UFRGS) from March 2015 to March 2018, according to the treatments performed on traumatized permanent teeth and the results after one year of follow-up. The sample was based on information collected from clinical records of 92 patients, referring to a) data before treatment, b) data on the diagnosis and proposed treatment, and c) data on the follow-up period. Descriptive and inferential statistical analyses were performed. Most patients were male (56.5%) and aged six to 11 (30.4%) and older than 18 (29.1%) years. The prevalent cause of trauma was falling, and 59% of patients were first assisted within 24 hours. The most frequent traumas were enamel and dentin fractures with and without pulp exposure (6.3%), lateral luxation (1.4%), and avulsion (1.9%). Endodontic treatment was usually performed on traumatized teeth (7.2%). Of the 92 patients, 58 returned for maintenance visits with a mean follow-up of one year. Most patients had successful treatments. Prompt care and patient follow-up were essential for the good prognosis of traumatized teeth.(AU)


O objetivo do presente estudo observacional retrospectivo foi analisar os casos atendidos na clínica de trauma dentoalveolar da Faculdade de Odontologia da Universidade Federal do Rio Grande do Sul (FO-UFRGS) no período de março de 2015 a março de 2018, de acordo com os tratamentos realizados em dentes permanentes traumatizados, assim como os resultados observados após 1 ano de seguimento. A amostra foi baseada na coleta de informações em prontuários clínicos de 92 pacientes, referentes a: a) dados anteriores ao tratamento; b) dados referentes ao diagnóstico e ao tratamento proposto; e c) dados referentes ao período de acompanhamento. Foi realizada análise estatística descritiva e inferencial. A maioria dos pacientes era do sexo masculino (56,5%), com idades entre 6 e 11 anos (30,4%) e mais de 18 anos (29,1%). A principal causa do trauma foi queda, sendo que 59% dos pacientes receberam o primeiro atendimento em 24 horas. Os traumas mais frequentes foram fratura de esmalte e dentina, com e sem exposição pulpar (6,3%), luxação lateral (1,4%) e avulsão (1,9%). O tratamento endodôntico foi frequentemente realizado nos dentes traumatizados (7,2%). Dos 92 pacientes, 58 retornaram para consultas de manutenção com um acompanhamento médio de 1 ano. A maioria dos pacientes apresentou sucesso após o tratamento. O atendimento no tempo correto, assim como o acompanhamento dos pacientes, foi fator fundamental para o bom prognóstico dos dentes traumatizados.(AU)

ABSTRACT

En la Facultad de Odontología de la Universidad de Buenos Aires, en la asignatura Clínica Integral para Niños y Adolescentes, los estudiantes del último año de la Carrera de Odontología realizan procedimientos clínicos y prácticos indispensables en la atención inicial de las urgencias en niños y adolescentes, restituyendo la integridad coronaria en lesiones producidas por traumatismos en piezas primarias y permanentes. Estudios publicados sobre la temática refieren que la mayoría de los estudiantes de pregrado no están lo suficientemente calificados, o tienen un conocimiento insuficiente para manejar situaciones de urgencias en traumatología dental. Sin embargo, se ha demostrado que existe un impacto positivo entre los estudiantes luego de recibir una clase teórica. El objetivo es determinar el nivel de conocimientos de los estudiantes de odontología del último año de la carrera en el manejo de los traumatismos dentoalveolares en niños y adolescentes, antes y después de recibir las clases teóricas (modalidad online) sobre esta temática (AU)

Objective: this study is designed to report a clinical intrusion case, which exhibited enamel hypoplasia on the crown of tooth 11 and which underwent clinical and X-ray monitoring for six years. Case report: A threeyear- old female patient exhibited intrusion of deciduous teeth 51, 52, 61, and 62. After X-ray, it was detected that teeth 51 and 61 had perforated the nasal cavity floor, but all four teeth had re-erupted and began to be monitored. After six months, teeth 51 and 61 exhibited bone loss in the interproximal area, as well as mobility, which led to tooth extraction. Teeth 52 and 62 re-erupted in a satisfactory position, and exhibited no signs and symptoms of infection, periapical alteration, or mobility. These teeth were therefore maintained in the arch. After six years, it was detected that tooth 11 had erupted with enamel hypoplasia. Final considerations: Longterm clinical and X-ray control proved essential, as it allowed for the early diagnosis of potential alterations, and minimized potential sequelae to the permanent teeth.(AU)


Objetivo: relatar um caso clínico de intrusão com acompanhamento clínico e radiográfico de 6 anos, que apresentou hipoplasia de esmalte na coroa do dente 11. Relato de caso: paciente de 3 anos, sexo feminino, apresentou intrusão dos dentes decíduos 51, 52, 61 e 62. Após radiografia, foi detectado que os dentes 51 e 61 tinham perfurado o soalho da fossa nasal, porém, ocorreu re-erupção dos 4 dentes e foi realizado acompanhamento. Após 6 meses, nos dentes 51 e 61, foram observadas perda óssea na região interproximal e mobilidade, optando-se pelas exodontias. Os dentes 52 e 62 re-erupcionaram em posição satisfatória e com ausência de sinais e sintomas de infecções, alterações periapicais e mobilidade, sendo mantidos no arco. Após 6 anos, foi observado que o dente 11 erupcionou apresentando hipoplasia de esmalte. Considerações finais: mostrou-se essencial o controle, clínico e radiográfico, em longo prazo, diagnosticando precocemente possíveis alterações e minimizando sequelas que podem acometer os dentes permanentes.(AU)

Objetivo: rizogênese incompleta refere-se a dentes permanentes em que não houve a formação completa do ápice radicular, ocasionando perda da vitalidade pulpar e, consequentemente, incapacidade da bainha epitelial de Hertwig para induzir a formação completa da raiz. Devido ao traumatismo e escurecimento dental, se faz necessário em certos casos a confecção de coroas em porcelana pura para reabilitar a paciente em estética e função. O presente trabalho tem como objetivo demonstrar um caso de reabilitação estética e funcional em dente com rizogênese incompleta, onde se consideraram as perdas estruturais e alteração de cor, para que seja possível estabelecer o tratamento mais adequado. Relato de caso: paciente de 16 anos, gênero feminino, atendida na Clínica de Odontologia de uma instituição privada, insatisfeita com a estética do seu dente devido ao escurecimento do elemento 21, pois quando tinha 5 anos o dente foi traumatizado juntamente com o elemento 11. O plano de tratamento adotado para ambos os elementos foi a confecção de peças protéticas em porcelana e cimentação. Considerações finais: esse caso demonstrou que a confecção de coroas estéticas em porcelana é considerada uma boa conduta na rotina clínica em casos de escurecimento dental, devolvendo estética e função para o paciente.(AU)


Objective: incomplete rhizogenesis refers to permanent teeth with incomplete formation of the root apex, causing loss of pulp vitality and the consequent inability of the Hertwig epithelial sheath to induce complete root formation. Due to dental trauma and darkening, some cases require the production of pure porcelain crowns to rehabilitate the aesthetics and function of patients. The present study aims to show a case of aesthetic and functional rehabilitation in a tooth with incomplete rhizogenesis, in which structural losses and color change were considered to establish the most appropriate treatment. Case report: a 16-year-old female patient treated at the Dental Clinic of a private institution. The patient was not satisfied with the aesthetics of her tooth due to the darkening of element 21, because at 5 years old the tooth was traumatized along with element 11. The treatment used for both elements was the production of prosthetic porcelain crowns and cementation. Final considerations: this case showed that the production of aesthetic porcelain crowns is considered a good conduct in the clinical routine in cases of tooth darkening, returning aesthetics and function to the patient.(AU)

Abstract This study evaluated clinical and radiographic twelve-month outcomes of root canal treatments (CT) with smear layer removal, performed in primary teeth, using two different root canal filling materials. Pulpectomy was performed on 27 primary teeth with necrosis or irreversible pulpitis, caused by dental caries or trauma, in 23 children (2-7 years old). A single trained operator performed the CT in a single visit in cases without periapical or interradicular radiolucency (PIR) or in multiple visits in cases with PIR. Participants were selected based on specific inclusion and exclusion criteria, and randomly allocated into two groups: Group 1 (G1) - iodoform paste (iodoform + camphorated parachlorophenol + ointment comprising prednisolone acetate 5.0 mg and rifamycin 1.5 mg); Group 2 (G2) - Calen®/ZO paste. Treated teeth were restored with composite resin immediately after the root canal filling. The outcomes were evaluated clinically and radiographically according to specific criteria. Two blinded and standardized evaluators assessed the radiographic outcomes. We used descriptive analyses due to the small sample size. CTs were performed due to caries lesions in 70.4% of the cases and due to trauma in 29.6%. Only one tooth of G1 was unsuccessful; hence, pulpectomy performance in both groups was not influenced by the filling material, nor by any other analyzed variable. The level of the root canal filling was better in the Calen®/ZO group. The clinical and radiographic twelve-month outcomes indicated successful treatment, independently of the root filling material used.

ABSTRACT

Abstract This study investigated risk factors for tooth injuries in individuals from a dental clinical reference service for patients with special needs in Belo Horizonte, MG, Brazil. This is a retrospective cohort study that evaluated 493 dental charts of individuals with or without tooth injuries at their first dental appointment. The dependent variable was the time of occurrence of new dental traumatic injuries and was measured in months. Gender, age, International Code of Diseases, mother's education, mouth breathing, hyperkinesis, pacifier use, thumb sucking, psychotropic drug use, tooth injuries at the first dental examination, involuntary movements, open bite, having one or more siblings and reports of seizures were the covariates. The Cox proportional hazards regression model was used to estimate the unadjusted and adjusted hazard ratios and their respective 95% confidence intervals. The average time that individuals remained free of dental traumatism was 170.78 months (95% CI, 157.89-183.66) with median of 216 months. The incidence of new events was 11.88%. The covariate associated with an increased risk of dental traumatism was a history of tooth injuries at the first dental appointment. The increase in dental trauma risk was 3.59 (95% CI, 1.94-6.65). A history of traumatic dental injury was the risk factor for the dental trauma found in this group of individuals with developmental disabilities.


Resumo Este estudo investigou os fatores de risco para traumatismo dentário em indivíduos da clínica odontológica de um serviço de referência para pacientes com necessidades especiais em Belo Horizonte, Brasil. Este é um estudo de coorte retrospectivo que avaliou 493 prontuários de indivíduos com ou sem traumatismo dentário em sua primeira consulta odontológica. A variável dependente foi o tempo de ocorrência de novas lesões traumáticas e foi mensurada em meses. ABSTRACT

Las edades más afectadas, por la presencia de fracturas complicadas de corona, son la niñez y la adolescencia. En la mayoría de los pacientes se afectan los incisivos centrales superiores permanentes de incompleta formación apical. Esto implica una disminución de las capacidades de masticación, dicción y trae consigo implicaciones psicológicas en la adolescencia. El tratamiento consiste en la aplicación de una técnica de apicoformación que dificulta lograr al unísono la resolución de la afección estética. Nuestro propósito es describir un caso de apicoformación en incisivo central superior izquierdo, con cementación de cánula rígida, conformación de muñón y restauración estético-funcional definitiva. Un paciente de 8 años, masculino acude a consulta tras sufrir traumatismo dentoalveolar consistente en fractura complicada de corona. Los rayos X muestran ligero ensanchamiento periodontal y ápice inmaduro. Se le aplicó anestesia local, se realizó la extirpación pulpar total del diente afectado, y se colocó medicamento intraconducto. La culminación del tratamiento de restauración total coronaria se realizó con la cementación con ionómero de vidrio tipo II de una cánula rígida No. 22 hasta las dos terceras partes de la longitud de trabajo; alrededor de esta se conformó un muñón y corona de nano composite híbrido fotopolimerizable. Se controló evolución cada 24 días, durante tres meses para comprobar el estado de maduración apical. La cementación de la cánula rígida, consituye una alternativa para el tratamiento funcional de la apicoformación por fractura complicada de corona. Supone una rápida recuperación de las funciones bucodentales y psicológico-sociales integrales del paciente(AU)


The life periods most commonly affected by complicated crown fractures are childhood and adolescence. The teeth most frequently affected are permanent upper central incisors of incomplete apical formation, resulting in limitations in chewing and speech, as well as psychological effects in adolescence. The treatment consists in the application of an apex formation technique which makes it difficult to also solve the esthetic effect. It was our purpose to describe a case of apex formation in the upper left central incisor, with rigid stem cementation, stump conformation and definitive esthetic and functional restoration. An 8-year-old male patient attends consultation after undergoing dentoalveolar trauma consisting in complicated crown fracture. Radiography revealed slight periodontal expansion and an immature apex. A local anesthetic was applied to perform total pulpal removal of the affected tooth and place intracanal medicament. At the end of the total crown restoration treatment, cementation of a No. 22 rigid stem was performed with type II glass ionomer up to two thirds of the working length, and a hybrid photopolymerizable nanocomposite crown and stump were shaped around it. Evolution was checked every 24 days for three months to verify apical maturation status until total formation was achieved. Rigid stem cementation is an alternative for the functional treatment of apex formation due to complicated crown fracture. It is characterized by a fast, complete recovery of the patients oral-dental and psycho-social functions(AU)

ABSTRACT

Introdução: a odontologia prioriza a manutenção e integridade dos dentes na boca e a ocorrência de traumatismos dentais vem dificultando o cumprimento desta meta, uma vez que suas consequências envolvem danos funcionais, estéticos, psicológicos, sociais e terapêuticos, além dos altos custos para a reabilitação e acompanhamento por longos períodos de tempo. Objetivo: avaliar o conhecimento dos cirurgiões dentistas da região metropolitana de Juazeiro do Norte sobre as condutas clínicas baseadas nos planos de tratamentos propostos por estes profissionais sobre fraturas coronárias com ou sem exposição pulpar. Método: a partir de um questionário descritivo, foram abordadas questões referentes ao perfil dos profissionais entrevistados e procedimentos empregados na conduta frente a fraturas coronárias. Setenta e cinco questionários foram devidamente preenchidos, sendo 34 mulheres e 41 homens. Resultados: dos entrevistados 13 relataram nunca terem atendido casos de trauma dental, 26% relataram serem incapazes de tratar todos os casos de traumatismo dentário. E mesmo a maioria tendo formação especializada, grandes dificuldades foram encontradas nos planos de tratamento propostos em relação a fraturas mais complicadas. Discussão: as lesões dentais traumáticas envolvendo tecidos duros e de sustentação podem representar padrões de cicatrização bastante complexos. As lesões traumáticas do tipo fraturas coronárias apresentam um prognóstico favorável para a manutenção da vitalidade pulpar em relação às fraturas coronárias complicadas. As fraturas corono-radiculares com ou sem envolvimento pulpar, foram as que apresentaram mais indicações inadequadas de tratamento, provavelmente por necessitarem de uma abordagem multidisciplinar. Conclusão: considera-se primordial a multidisciplinaridade durante a formação acadêmica do clinico geral, bem como o aperfeiçoamento ao atendimento a pacientes com trauma dental. (AU)


Introduction: Dentistry prioritizes the maintenance and integrity of teeth in the mouth and the occurrence of dental trauma is hampering the achievement of this goal, since its consequences involve functional, aesthetic, psychological, social and therapeutic damage, in addition to high costs for rehabilitation and monitoring for extended periods of time. Objective: The objective of this study was to assess the knowledge of dentists in the metropolitan region of Juazeiro do Norte on the clinical management based on treatment plans proposed by these professionals on crown fractures with or without pulp exposure. Methods: For that, starting from a descriptive questionnaire, some questions regarding the profile of inquired professionals and procedures used in the performance of treatment of crown fractures. Results: Seventy-five questionnaires were fully completed, 34 women and 41 men. Only 13 reported ever having met cases of dental trauma, 26% reported being unable to deal with all cases of dental trauma. And even most having specialized training, great difficulties were encountered in the proposed treatment plans for the most complicated fractures. Discussion: Traumatic injuries involving pulp and periapical tissues can represent very complex healing patterns. The crown fractures have a favorable prognosis for the maintenance of pulpal vitality in relation to coronary complicated fractures. The corono-root fractures with or without pulp involvement, were the ones that had more inadequate treatment, probably because they require a multidisciplinary approach. Conclusion: it is essential for multidisciplinary academic training of general practitioner as well as improving service to patients with dental trauma. (AU)

ABSTRACT

Las fracturas de la raíz también llamadas intra alveolares radiculares, son aquellas que afectan a la dentina, al cemento y la pulpa, se caracterizan por presentar patrones complejos de curación, debido al deterioro de la pulpa, el ligamento periodontal, la dentina y el cemento. La mayoría de estas fracturas se producen por un golpe directo contra el suelo, un cuerpo extraño que incida sobre el diente o accidentes automovilísticos y deportivos. En el presente artículo se describe el tratamiento y seguimiento a 2 años 4 meses del caso de una niña de 10 años de edad que después de un traumatismo fue sometida a un tratamiento no adecuado, que le causaba mucho dolor, radiográficamente se observó la ausencia del tercio apical en el diente 21 y una movilidad grado III del mismo. Se modificó el tratamiento prolongando la vida del diente en boca. (AU)


Root fractures also known as intraalveolar root fractures, are those which involves and damage the dentin, cement and pulp, they are often characterized by the presence of weird and unusual healing patterns, mainly because of the pulp, periodontal ligament, dentin and cement injuries. Most of this fractures are caused because of a direct traumatism against the ground, also by any other object that directly hits the thoot, or any type of sports or car accidents, In this article its described the treatment and a follow up for 2 years 4 months of a 10 year old traumatized girl, which after the accident was treated in the wrong way causing her a lot of pain and periodontal problems, radiographically it's shown the abscense of the apical third of tooth 21 and an advanced movility degree. The treatment was drastically modified achieving the main purpose wich was to preserve the tooth in function. (AU)

ABSTRACT

Los implantes osteointegrados permite recuperar de forma fija y cómoda todas las funciones del diente perdido. El objetivo de esta presentación es demostrar los beneficios de realizar tratamientos ortodóncico previos a la colocación de implantes dentales en pacientes edentes. Un paciente de 21 años acudió a consulta de implantología en la Clínica Estomatológica Provincial Docente Dr. Ismael Clark y Mascaró; de Camagüey, por ausencia de incisivos central y lateral superior derecho (11 y 12). Se valoró por el equipo multidisciplinario, el cual planteó cerrar el espacio de la brecha edente con aparatología ortodóncica fija y después realizar cirugía de avanzada y la colocación de un implante Leader Nano. La rehabilitación protésica se realizó a los ocho meses. Se demostró la eficacia de la realización de tratamiento ortodóncico previo a la colocación de implantes dentales así como el restablecimiento del estado estético y psicológico del paciente(AU)


Osseointegrated implants make it possible to recover all the functions of the lost tooth in a comfortable and permanent manner. The purpose of this presentation is to show the benefits of applying orthodontic treatment before the placement of dental implants in edentulous patients. A 21-year-old male patient attended implantology consultation at Dr. Ismael Clark y Mascaró Provincial Teaching Dental Clinic in Camagüey, due to the absence of the right upper central and lateral incisors (11 and 12). Upon assessment, the multidisciplinary team decided to close the toothless gap with a fixed orthodontic appliance and then perform advanced surgery and the placement of a Leader Nano implant. Prosthetic rehabilitation was performed eight months later. It was shown that performing orthodontic treatment before the placement of dental implants has a positive effect on the recovery of the esthetic and psychological status of patients(AU)

ABSTRACT

Regenerative endodontic procedures are biologically based procedures which deal with the regeneration of pulp‑like tissue, more idealistically the pulp‑dentin complex. The regeneration of this pulp‑dentin complex in an infected necrotic tooth with an open apex is possible only when the canal is effectively disinfected. Though there are various procedures for treating open apex ranging from Ca(OH) 2 apexification, mineral trioxide aggregate apexification and surgical approach, regeneration of tissues has always taken superior hand over the repair of tissues. The mechanics behind the regenerative endodontic procedures is that despite the tooth being necrotic, some pulp tissue can survive apically which under favorable conditions proliferate to aid in the process of regeneration. In the past 2 decades, an increased understanding of the physiological roles of platelets in wound healing and after tissue injury has led to the idea of using platelets as therapeutic tools in the field regenerative endodontics. In the present case report with an open apex, high sterilization protocol is followed using triple antibiotic paste as intra‑canal medicament, followed which platelet rich fibrin is used as the regenerative material of choice. Over an 18‑month follow‑up period, clinically patient is asymptomatic and radiographically there is complete regression of the periapical lesion and initiation of the root end closure.

ABSTRACT

OBJECTIVE: To assess the level of knowledge of primary school teachers in the public school network of Northeastern Brazil with respect to management of dental trauma and its relationship with prognosis. METHODS: A questionnaire was applied to 195 school teachers of public schools in Northeastern Brazil. The questionnaire comprised 12 objective questions about dental trauma and methods for its prevention and management. Data were submitted to chi-square test and Poisson regression test (P > 0.05). RESULTS: Out of the 141 teachers who responded the questionnaires, the majority were women (70.2%) and most of them had experienced previous dental accidents involving a child (53.2%). The majority (84.4%) had incomplete college education and few were given some training on how to deal with emergency situations during their undergraduate course (13.5%) or after it (38.3%). Their level of knowledge about dental trauma and emergency protocols showed that unsatisfactory knowledge level was associated with the male sex: 46% higher for men in comparison to women (P = 0.025). CONCLUSIONS: Approximately half of teachers evaluated had unsatisfactory knowledge about dental trauma and emergency protocols, with female teachers showing more knowledge than men. .


OBJETIVO: avaliar o nível de conhecimento de professores de escolas primárias na rede pública de ensino na região nordeste do Brasil, no que diz respeito ao manejo do paciente traumatizado e sua relação com o prognóstico. MÉTODOS: aplicou-se um questionário a 195 professores da rede pública de ensino da região nordeste do Brasil. O questionário continha 12 questões objetivas sobre os traumatismos dentários e seus métodos de prevenção e manejo. Após obtenção dos dados, esses foram submetidos ao teste qui-quadradro e ao teste de regressão de Poisson (p > 0,05). RESULTADOS: dos 141 professores que responderam o questionário, a maioria era composta pelo sexo feminino (70,2%) e a maior parte já vivenciara algum episódio de acidente com criança (53,2%). A maioria (84,4%) possui ensino superior incompleto e poucos foram contemplados com manobras de urgência na graduação (13,5%) ou após essa (38,3%). Os resultados da regressão para o nível de conhecimento sobre traumatismos dentários e protocolo de urgência em relação às variáveis explanatórias do estudo demonstraram que o conhecimento insatisfatório foi associado ao sexo masculino, sendo 46% maior para os homens (p = 0,025). CONCLUSÃO: aproximadamente metade dos professores possui conhecimento insatisfatório sobre traumatismos dentários e protocolo de urgência, sendo que os professores do sexo feminino apresentaram maior conhecimento do que os do sexo masculino. .

Se presenta el caso de un niño de 2 años de edad que es llevado a la consulta de emergencia por la madre, manifestando que su hijo sufre una caída por estar corriendo en el patio de su colegio, golpeándose los dientes anteriores. El paciente llega a la clínica odontológica de la UPCH, donde al examen clínico y radiográfico de las piezas 51,61 presentaron diagnóstico de fractura coronaria complicada las que recibieron tratamientos de pulpectomía y luego fue rehabilitado con coronas celuloide y resina compuesta; la pieza 52 presentó fractura coronaria no complicada la que recibió como tratamiento una restauración de resina compuesta, se reporta el presente caso para indicar la alternativa de tratamientos que restablecen la funcionabilidad y estética...


This paper reports the case of a 2 year old boy who was taken to the emergency room by his mother stating that her child fell forward after being running in the school yard, resulting in anterior teeth trauma. The patient was taken to the UPCH Dental Clinic, where, after the clinical and radiographic examination, the 51 and 61 teeth resulted with complicated crown fracture receiving both pulpectomy treatments and then they were rehabilitated with strip crowns and composite resin; tooth 52 presented uncomplicated crown fracture and received a composite resin restoration treatment. Introdução: o presente Caso Clínico se refere a dois dentes, de um único paciente, que foram, inicialmente, apenas imobilizados com uma contenção semirrígida por 15 dias e sem intervenção endodôntica. Objetivo: descrever o caso de dois dentes acometidos por luxação extrusiva e tratados por meio de uma pasta obturadora composta por hidróxido de cálcio, clorexidina gel a 2% e óxido de zinco. Métodos: após o acompanhamento de um mês, notou-se que os dentes não apresentavam vitalidade pulpar e que, radiograficamente, demonstravam sinais de reabsorção externa. Foi realizado o tratamento endodôntico e a utilização desse novo protocolo terapêutico de medicação intracanal, que pode ser concluído em uma única sessão, e com possibilidade de permanência por até quatro anos no conduto radicular. Resultados: no presente relato, a pasta obturadora permaneceu no canal radicular, sem necessidade de troca, por 24 meses, com regressão das lesões periapicais e paralisação das reabsorções inflamatórias. Conclusão: diante disso, pode-se concluir que essa pasta obturadora pode demonstrar sucesso e efetividade para o tratamento de dentes traumatizados com reabsorção radicular.

ABSTRACT

Traumas dentários envolvem dentes anteriores, influenciando função e estética de crianças e adultos jovens de diversas idades em diferentes especialidades odontológicas. Os dentes mais frequentemente envolvidos são incisivos centrais superiores, incisivos laterais superiores e incisivos centrais inferiores. A proposta deste trabalho foi apresentar caso clínico de traumatismo envolvendo fratura óssea em bloco. Após acidente o paciente foi atendido e ao exame clínico constatou-se fratura em bloco dos elementos dentais 42,41,31,32 e avulsão do 33 com tecido gengival e mucoso dilacerados. Radiograficamente observou-se fratura horizontal do osso mandibular, avulsão dos dentes de seus alvéolos em bloco e diversos fragmentos ósseos. Seguiu-se cirurgia imediata, valendo-se de contenção emergencial com microparafuso e após 7 dias removeu-se a sutura. Realizou-se acompanhamento em 14,21 e 28 dias e após 3 meses foi realizado tratamento endodôntico dos dentes 31, 32, 41 e 42. Os procedimentos realizados alcançaram êxito após a realização dos atendimentos.


Dental traumas involve anterior teeth, affecting both function and aesthetics of children and young adults of different ages in different dental specialties The teeth most frequently involved are the maxillary central incisors, lateral incisors and mandibular central incisors. The airn of this work is to present a clinical case of injury involving bone block fracture. After the accident, the patient underwent clinical examination, which revealed block fracture of dental elements 42, 41, 31, 32 and avulsion of 33 with laceration of the gingival and mucosal tissue. Radiographic results revealed horizontal fracture of the jawbone, tooth avulsion of its wells in the block and several bone fragments. Patient underwent immediate surgery using emergency containment with micro-screws, and suture was removed seven days after. Monitoring was performed at 14,21 and 28 days and endodontic treatment of teeth 31,32,41 and 42 was performed 3 months after. Ali procedures were successful.
